Provides a Medically Tailored Meal community support program serving Medi-Cal patients across California. Contracted by CalAIM to benefit Contra Costa Health Plan members, and provides 12 weeks of Medically Tailored Meals at no cost to individuals with chronic conditions. Offers a nutrition assessment by a registered dietitian who can prescribe 2 meals per day for 12 weeks. Provides three types of meals which include: fully prepared meals, semi-prepared meals and food boxes. Meals are delivered to clients home address and clients can select from a diversity of flavors and cultures.
