Provides permanent affordable housing solutions and vital support services to homeless and very low-income families and individuals in Contra Costa County. All Permanent Supportive Housing units are filled through the CC Coordinated Entry system. Homeless individuals can be placed on the Coordinated Entry list at the CARE Centers.
Provides on-site services at three Affordable Housing apartment complexes: Lakeside Apartments (Concord), Los Medanos Village (Pittsburg), and Bella Monte Apartments (Bay Point) accept housing applications at each of those sites.
