Services include: care-planning assessment, determination of eligibility for public assistance, eviction protection, long-term planning and budgeting, information and referral, elder abuse prevention and investigation.

Meals on Wheels Diablo Region
Provides Home Delivered Meals with daily wellness checks, Cafes, Health and Wellness classes, Friendly Visitors, Friendly Callers, and Friendly Helpers, Grocery Bags, Fall Prevention services, Care Management and Outreach, and Breakfast Bags. Meals are delivered around the noon hour by volunteers, 5 days a week. If weekend or holiday meals are desired, frozen foods can be delivered ahead of time. Includes wellness check.
