Description

Provides care management services to help older adults and their families meet long-term care needs. Services include: care-planning assessment, determination of eligibility for public assistance, eviction protection, long-term planning and budgeting, information and referral, elder abuse prevention and investigation. Provides a one-time assistance to pay rent or utilities.

Meals on Wheels Diablo Region

Offers a hot meal delivery program for frail and home bound persons 60+. Regular meals, fruit dessert; special diet needs include restricted sodium. Meals are delivered around the noon hour by volunteers, 5 days a week. If weekend or holiday meals are desired, frozen foods can be delivered ahead of time. Includes wellness check.
