Application process

Residents are referred through the CC County Coordinated Entry System. This system can be accessed through the Care Centers, shelters, or by dialing 2-1-1 if physically unable to access a CARE Center. Los Medanos Apartments (Pittsburg), Bella Monte Apartments (Bay Point), Lakeside Apartments (Concord) have on-site application processes.

Provides permanent affordable housing solutions and vital support services to homeless and very low-income families and individuals in Contra Costa County. All Permanent Supportive Housing units are filled through the CC Coordinated Entry system. Homeless individuals can be placed on the Coordinated Entry list at the CARE Centers. Provides on-site services at three Affordable Housing apartment complexes: Lakeside Apartments (Concord), Los Medanos Village (Pittsburg), and Bella Monte Apartments (Bay Point) accept housing applications at each of those sites.

Offers multi-site permanent supportive housing for chronically homeless adults and families throughout the county.
