Street Outreach Programs | Coordinated Outreach Referral and Engagement (CORE) - CCHS
Provides outreach to homeless encampments by distributing clothing items, minor medical care and general hygiene products. Refers individuals to services including food and linkages to healthcare. Provides assessments to homeless individuals or families who are unable to get to a homeless CARE Center or individuals age 65+ and inputs their information into the County Homeless Coordinated Entry System. Website has extensive information with health guidance from the health director and Frequently asked questions. Answers dispatch line for Concord shelter, and East County Interim Housing Program, Concord Warming Center and GRIP Warming Center. Offers limited transportation to CORE clients only.
