Application process
Call for services, home visit appointment through telehealth will be scheduled for an assessment. Tour of local center and enrollment for eligible clients by appointment.
Fees
Accepts Medi-Cal, Medicare, and private pay.

Offers a health plan for older adults who want to stay living at home but need extensive home care and help to get to their doctor. The centers have clinics, gyms and day centers. Offers full health coverage, homecare and/or adult day center.
