Eligibility
Must be the dog's primary caretaker and owner, responsible for paying the dog's medical care, diagnosis from veterinarian and must be providing the cancer treatment. Client must give permission for Magic Bullet Fund to contact veterinarian for information. Additional eligibility requirements for dog’s weight are online.
Required documents
Most recent 1040 form or disability letter/stub, previous month bank statements, car registration, invoice from clinic where dog will receive cancer treatment and proof of financial hardship.
