Wraparound Facilitation/Community Support | HOPE - Lincoln Families
Provides integrated behavioral support services for students in K-12. Develops tools and learning environments for academic success across all levels from teacher consultation, student counseling and whole school supports. Provides comprehensive support including wraparound service with an array of services including, in-class/playground support (elementary level), individual, group and family therapy, teacher consultation, crisis intervention and community outings.

Application process
Counselors, teachers, or administrators can submit a referral to the COST/CARE team at their school site. Parent or guardian can call to self refer their child.

Medi-Cal number or social security number required in order to evaluate students eligibility.
Eligibility
K-12 student enrolled in Pittsburg Unified School District. 6-8th grade students at Antioch Middle, Dallas Ranch Middle, or Park Middle School in Antioch Unified School District. Must have full scope Contra Costa Medi-Cal.
