Clubhouse Model Psychiatric Rehabilitation | Connections House

Provides vocational and social rehabilitation in a supportive environment for Contra Costa County adults living with serious mental illness. Participants share ownership and responsibility for the success of the organization by working in a unique partnership with a small staff, building on strengths rather than focusing on illness and finding purpose and meaning through work. Provides a safe, accepting place to spend the day, valuable work to perform within the organization, opportunities to socialize with friends and co-workers, and access to employment and resources within the wider community. Membership is free and lifelong. Members are eligible for supportive services including, rental assistance, food and transportation. Site offers a food pantry for members who are facing food insecurities and supports members in signing up for CalFresh and connecting to local food banks.

Eligibility

Resident of Contra Costa County (when applying), age 18+, with one of the following diagnoses: Schizophrenia, Schizoaffective Disorder, Bipolar Disorder, Major Depressive Disorder, Generalized Anxiety Disorder, or PTSD. Can self refer, but a psychiatrist, case manager, therapist, or other healthcare provider will be required to complete an enrollment form.
