California resident (legal residency not required) with qualifying developmental disability, or requesting evaluation for the possibility of disability. Does not offer speech services for communication concerns if over age 3, clients should contact their child's school district. Individuals over 3 must have a concern in Autism, Epilepsy, Cerebral Palsy, and Intellectually Disability. There is no citizenship requirement.

For children under 3 call 510-618-6195. If no answer, leave voice message with name, phone number, child’s name and date of birth, parents name, address and child concerns. Call will be returned within 48 hours. Process can take up to 45 days from initial referral. For children over 3 and adults call 510-618-6122, process can take up to 120 days.

No fee for Medi-Cal recipients; dependent on income, some services for consumers under 18 years require family cost participation.

Description

Provides case management services for individuals with developmental disabilities and infants and toddlers with developmental delays and those at high risk for developmental disabilities. Works in partnership with individuals and other agencies to plan and coordinate services and supports. Services include information and referral, assessment and diagnosis, identification and assistance in finding and using community and other resources, family support, early intervention services for eligible infants and toddlers with developmental disability or significant developmental delay. Family Transition Coordinator reaches out to families who were with RCEB for 0-3 but for various reasons did not continue services and there may still be developmental concerns.
