Application process
Call a minimum of 5-10 business days notice prior to appointment date and time to arrange transportation to allow sufficient mail time to send materials such as tickets and paperwork through certified mail.

Provides non medical transportation to Contra Costa Health Plan members who need a transportation to get to a medical appointment, specialty clinic or pharmacy. Health Plan makes transportation reservations i.e. Taxi, Uber or mails public bus transportation tickets including Paratransit tickets to members.
