Application process
Submit application online or call to have form mailed. Once the application has been successfully processed, CPR will mail a paper copy, to the patient. The paper copy must be signed by the patient, and returned along with all requested supporting documentation. This is required before the patient will be considered for eligibility for the Co-Pay Relief program.
