60+, Contra Costa resident, uninsured or on Medi-Cal or on Medi-Cal and Medicare combined. Excluded from service are individuals on Medicare only or with private insurance or with a primary dementia diagnosis.

Description

Provides mental health services for adults 60+ with a qualifying mental health concern. The service is provided at the individuals home or community location by a mobile team of clinicians. Eligibility requires client be uninsured or on Medi-Cal or on Medi-Cal and Medicare combined. Excluded from service area individuals on Medicare only or with private insurance or with a primary dementia diagnosis.
