Adults age 26-49, residing in Contra Costa County, with no other health insurance and income must be below 138% of the federal poverty line.

Application process
Clients must enroll under health center of choice. Clients who wish to receive medical care at a different health center facility, must complete a new CARES application.

Contra Costa County
Provides free primary care coverage to adults living in Contra Costa County that do not qualify for other health insurance. The program assigns members to a primary care home. Enrolled participants have access to preventative care services, free of charge, at either Brighter Beginnings, La Clinica de La Raza, or LifeLong Medical Care. There are no monthly premiums or copays.
