Provides information, referrals, assistance, understanding and hope to persons of all ages with disabilities as well as elderly individuals for the purpose of helping them to live as independently as possible. Services include: advocacy, benefits assistance, housing assistance, employment services, peer counseling services, assistance with independent educational planning (IEPs for school-aged youth), life skills, ADA information, public & businesses education. Referrals and resources are provided for: assistive technology, agencies serving the disabled, transportation, and youth programs. Advocacy services to increase access to services, benefits, and other social services and advising clients regarding their rights. Housing referral services assist clients by maintaining a registry of accessible, adaptable, affordable apartments and houses. Peer counseling is available to disabled individuals, their families, and friends. In depth individual skills development can be provided within the peer-counseling group or on a one-to-one basis. A small lending library of equipment is available.

Provides current Contra Costa County Medi-Cal and CalFresh clients with assistance reporting Medi-Cal and CalFresh changes (such as a new address, income, newborn, etc.), answers about benefits, and assistance completing forms. Offers benefit information for dental care.

Provides legal services via remote advocacy. Housing, DV Restraining Order and Consumer clinics are all suspended; contact hotlines for assistance. Offers free legal services related to health care access and coverage, including Covered California, public and private insurance and other health care access related issues. Issues included but not limited to: Medi-Cal and Denti-Cal, Medicare, medical debt, IHSS, paratransit services, mental health services and county indigent programs.

Offers Mind, Body, and Soul support group to East African American individuals. Host four support groups that meet twice a month on a rotating basis, to follow social distancing. Community health advocates help individuals with health, mental health, and basic needs navigation and assistance.

Provides services for Medi-Cal members that address social determinants of health (SDOH). SDOH are things such as food, housing, and transportation. CCHP offers Community Supports free of charge, to help CCHP Medi-Cal members.

Provides advocacy and community education for individuals in long-term care facilities including assisted living, skilled nursing, board and care. Provides information by phone or on website on for individual skilled nursing facilities by specific types of care, Medi-Cal information, estate planning and referrals to elder care lawyers; also provides extensive information on choosing a facility, assessment tools, etc.

Provides resource referral, navigation and benefits application assistance. Benefit assistance includes CalFresh, Medi-Cal, CalWORKS, General Assistance, and Cash Assistance Program for Immigrants (CAPI).

In collaboration with Contra Costa County, Supervisor Burgis Office, and City of Antioch, provides a one day festive event that offers a wide variety of on-the-spot government services at one single location. Clients can apply for CalFresh and MediCal, gather information about childcare and preschool options, record with Reach from Clerk-Recorder's Office, obtain a Covid-19 vaccine, sign-up for library cards, obtain weatherization information, register to vote, and obtain clean slate program information.
