Foster Homes for Dependent Children | Agape Villages Foster Family Agency

Provides a safe home and treatment programs for children with behavioral and emotional challenges. Children, ages birth-21, are referred by the Department of Social Services, Child Protective Services or one of the county mental health departments. Treatment services take place in a treatment foster home with emphasis in developing and maintaining competent, healthy, committed and trained treatment foster families in which the foster children may be nurtured and redirected. Requirements for potential foster families are listed on website or call for information.

Adoption Information/Referrals | Children & Family Services - Pleasant Hill - CCC EHSD

Provides Concurrent Planning, a program which place children with a potential adoptive home while continue to work with the birth family, providing reunification services. Concurrent caregivers are foster parents and prospective adoptive parents at the same time, offering stability while reducing their number of placements. Families must commit to provide permanency while at the same time partnering with the birth parents and CFS in Reunification services. Families interested in fostering or adopting must first attend an orientation. Orientations provide information of policies, and types of children needing families are discussed in detail.

Case/Care Management | Regional Center of the East Bay - Contra Costa Office

Provides advocacy, client program planning, coordination of generic services, and consultation. Provides case management services through its own units and through two other contracted delegate agencies, La Familia Counseling Services and Asian Community Mental Health Services. An active client of the Regional Center must participate, at a minimum, in planning meetings to develop or revise an Individual Program Plan (IPP), or an Individual Family Service Plan (IFSP) for a child under 3 years of age. An IPP identifies services and plans necessary for a client to achieve agreed upon objectives.

Case/Care Management | Young Adult Program - Trinity Center

Case/Care Management | Transition Age Youth (TAY) Partnership - Fred Finch Youth and Family Services

Provides community-based comprehensive range of intensive support services and housing referrals in Contra Costa County for transition age young adults ages 16 - 25 that have significant mental health issues and are homeless or at imminent risk of homelessness. This includes young adults who are currently undocumented immigrants. TAY's purpose is to help youth to be able to achieve their individualized, recovery-focused goals and to be able to function as self-sufficiently as possible within their homes and communities. The program supports youth with accessing a variety of housing referrals. TAY is a collaborative partnership of the Fred Finch Youth Center, Contra Costa Youth Continuum of Services, Greater Richmond Interfaith Program's and The Latina Center, as well as Contra Costa County Mental Health Services.

Family Maintenance/Reunification | Foster Family Program - Northern California Family Center

Recruits and provides foster family certification to enable families to be eligible to provide a crisis home for youth needing a safe environment for youths, ages 0 to 17, who are experiencing crisis while NCFC counselors work with the youth and family to reunite them, or develop mutually agreed upon alternatives. Also provides Long-term Foster care for children referred by Social Services in Contra Costa, Alameda, and Solano counties. To be eligible, family must possess the ability to relate effectively to and adequately supervise youth, have space to temporarily house one or more young people, possess the ability to be certified by the NCFC as a Foster Family, and pass child abuse and criminal background checks.

Collaborates with the City of Richmond, other public agencies, nonprofit partners, and civic and private groups for outreach and encampment organizing among unsheltered people across West Contra Costa County. Offers wellness center which provides basic needs services including access to shower, laundry, food, clothing, water, transportation and connections to community resources, and outreach and engagement including trash cleanup. Provides workforce clinic, free classes, for resume and cover letter preparation, and job readiness as well as job search assistance.
