Provides Enhanced Care Management (ECM) services for Medi-Cal members with highly complex needs. ECM is a benefit that provides extra services to help clients get the care needed to stay healthy. Qualified individuals will have their own care team, including a Lead Care Manager who will talk to client, doctors, specialists, pharmacists, case managers, social services providers and others to make sure everyone works together to get client the care needed. Lead Care Managers can also help clients find and apply for other services in their communities which include, medically tailored meals and housing supports.

Provides assistance to previously incarcerated individuals to apply for public benefits, housing, employment, and medical appointments within a community medical clinic named Transitions Clinic.

Provides treatment to families who are reunifying after separation resulting from domestic violence, divorce and or child abuse. Offers family therapy which assists blended families and divorced parents who are attempting to parent cooperatively. Provides specialized couples and family therapy to families of offenders who wish to reunify.

Provides resources for relatives raising the child of a family member who need assistance with meeting basic needs. Connection with community resources will occur prior to providing the family with assistance from the program.

Recruits and provides foster family certification to enable families to be eligible to provide a crisis home for youth needing a safe environment for youths, ages 0 to 17, who are experiencing crisis while NCFC counselors work with the youth and family to reunite them, or develop mutually agreed upon alternatives. Also provides Long-term Foster care for children referred by Social Services in Contra Costa, Alameda, and Solano counties. To be eligible, family must possess the ability to relate effectively to and adequately supervise youth, have space to temporarily house one or more young people, possess the ability to be certified by the NCFC as a Foster Family, and pass child abuse and criminal background checks.

Provides outpatient adolescent mental health counseling to Contra Costa County residents ages 0-17 with full scope Medi-Cal. Does not provide psychiatric services.

Provides services which focus on health assessments to identify communicable diseases and to refer those in need of medical attention to health care providers. Working in collaboration with different agencies, assists refugees transition to life in America and become self-sufficient. Services include outreach, health screenings, health education, immunizations, tuberculosis follow-up and care, referrals to health and human services and interpretation.

Provides a variety of recreational and educational classes to seniors. Delivers hot and cold lunches to home-bound seniors living in the surrounding community on Monday, Wednesday, Thursday and Friday. Volunteers make weekly friendly reassurance calls.

Offers geriatric assessments which provide personalized aging consultation and assessment to seniors and their families. Geriatric social workers evaluate physical health, mental health, medical history, social connection, family support, and home safety. Works with the family to create a care plan, outlining recommendations and providing resources to address each client's particular challenges.

Provides resources and support for relatives in a group setting. Offers educational forums which are held in collaboration with local community colleges.

Offers an evidence-based long-term home visiting program that helps mothers with their first child. Due to the pandemic all visits are held online. Partners mothers with Public Health Nurses early in their pregnancy and offers two to four home visits per month that continue through her child’s second birthday. Public Health Nurses provide education to help women develop positive health behaviors during (and after) pregnancy, parenting education to support child health, development, and safety. Offer nursing assessments for early identification and monitoring of maternal and infant health status, encourages self-sufficiency by supporting parents attainment of their education and employment goals. Promotes supportive relationships among family and friends makes referrals to health and social service resources, in addition to care coordination with health and social service providers.
