Provides rapid response, legal services, and community education project to support safety and justice for immigrant families in Contra Costa County. In partnership with Contra Costa County, and community-based organizations, provides culturally competent, no-cost rapid response support, legal defense services, and immigrant rights education and training to support families impacted by anti-immigrant policies and practices affecting Contra Costa County residents.

Provides a therapist referral service of gay, lesbian, transgender and bisexual therapists via online, offers seminars, consultations, and speakers by members on such LBGTQ-related topics as homophobia, coming out, gay marriage, HIV/AIDS, alternative families, parenting, gender identity, S/M/B/D, as well as how anxiety and depression affect our community.

Provides a supported living program for adults age 20+ with Autism Spectrum Disorders and neurodevelopmental disabilities. Clients work toward self-reliant living and learn to advocate for themselves through a wide variety of support offered at the center.

Supports young adults, between the ages of 18 and 28, who experience or are at risk of experiencing housing and food insecurity. Focuses on chronic homelessness prevention in the youth community. Engages in community outreach and provides rapid resolution and chronic homeless prevention. Service includes, case management, job finding assistance, and resume preparation etc.

Provides Ombudsmen who advocate for residents of long-term care facilities which include skilled nursing facilities, large assisted living facilities and small 6-8 bed residential care homes for the elderly. Receives complaints and concerns from family members of patients and residents themselves, concerned friends and citizens, acute hospital staff and paramedics and government and private agencies. Ombudsmen work with residents to improve their quality of care thru review of care plans and mediating with health care providers. Also, functions as the official reporting and investigation agency for elder and dependent adult abuse incidents occurring in long-term care facilities.

Provides legal representation for indigent persons accused of crimes in Contra Costa County. Offers assistance with clearing prior criminal records through the Clean Slate Program. Provides rapid response, legal services, and community education project to support safety and justice for immigrant families through the Stand Together Contra Costa Program. Represents youth in long-term placement (group homes) and long term in-custody programs as well as youth in extended foster care proceedings through the Juvenile Justice Program. Provides legal assistance, information, and representation to individuals who have been arrested or cited for misdemeanor offenses from the time that they have had police contact through the Early Representation Program. Provides interdisciplinary case management and navigation services to individuals to ensure timely and coordinated access to a client-centered array of housing, behavioral health, transportation, and legal services at time of initial law enforcement contact. Provides community-based solutions to programs for alternatives to custody and develop re-entry plans by communicating directly with clients and members of their support systems. Works with clients to identify alternatives to incarceration such as substance abuse treatment, mental health treatment or connections with re-entry case management programs throughout Contra Costa County. Provides incarcerated, convicted, and sentenced individuals access to different kinds of post-conviction relief and resentencing, made available through recent state legislation.

Provides door-to-door transportation services for ADA certified individuals unable to use AC Transit buses or BART trains. Service is available from Monday-Sunday, mirroring established BART trains or AC Transit bus line times and routes. ADA certification is required for paratransit service. Provides emotional support for partners and spouses who are mourning the death of their significant other. Support group is facilitated by trained volunteers.

Provides assistance to patients who have been living in skilled nursing facilities return home or to community living. Explores available options, services and resources including; home and vehicle adaptation, home set up, assistive devices, wheelchairs and other equipment, training on self-care, nursing visits and how to get a personal care attendant.

Provides a variety of activities for LGBTQ youth including a drop-in time, social and support groups for LGBTQ youth. Weekday and Saturday activities, special events as well as a support group for parents & guardians of trans and gender nonconforming children.
