Partners with the Contra Costa County Employment & Human Services Department (EHSD) and the County Health, Housing and Homeless Services Department (H3) to provide the HousingWORKS! Housing Support Program (HSP): HousingWORKS! provides supportive housing services to homeless CalWORKs families to help them achieve self-sufficiency and housing stability. Supportive services include assistance with housing search and temporary/permanent housing placement, time-limited subsidy assistance using Rapid Rehousing and Housing First models and on-demand in-home services. Eligible families are referred through EHSD and the H3 County Coordinated Entry System. Offers Youth and Young Adult (YYA) Housing Support program which provides time-limited subsidy assistance and in-home support services to transition age youth to help them achieve permanent housing and independence the CC County Coordinated Entry System.

Supports young adults, between the ages of 18 and 28, who experience or are at risk of experiencing housing and food insecurity. Focuses on chronic homelessness prevention in the youth community. Engages in community outreach and provides rapid resolution and chronic homeless prevention. Service includes, job finding assistance, and resume preparation etc.

The program serves women ages 18 and older who are in recovery, re-entry after incarceration, or transitioning out of homelessness or unstable housing. Legacy Outreach provides a safe, structured, substance-free shared living environment where residents can rebuild stability, develop accountability, and work toward long-term independence. The home is designed for women who are motivated to participate in a respectful sober living community and who can safely live in a shared housing setting. The program emphasizes dignity, structure, personal responsibility, and community support.

Provides information about city's housing assistance programs, projects, and available resources. Offers information about Concord Rent Stabilization and Just Cause for Eviction including rent registry. Answers questions related to housing for tenants and landlords.

Provides veterans financial assistance to entry of new housing or eviction prevention and Utility Assistance. The program also has potential to assist with past eviction debt on a case-by-case basis if it is a barrier to entry of new housing.

Works with individuals and families who are experiencing literal homelessness. Provides housing problem-solving and/or one time financial assistance to assist individuals in exiting the homeless system of care. Financial assistance includes security deposit and housing relocation assistance.

Offers temporary housing by providing motel vouchers, transportation, relocation funds and assistance with mediation. Offers assistance in locating permanent housing and supports those at risk of homelessness.

Offers assistance is to help prevent homelessness for households at immediate risk of becoming homeless, once every two years. Program provides limited financial assistance to eligible applicants for housing-related expenses. Assistance may be available for up to one month of rental arrears (back rent) for households that can demonstrate the ability to maintain ongoing housing costs after receiving support. Security deposit assistance may also be available for individuals or families who are relocating due to an unexpected or urgent situation, with requests required to be submitted within 30 days before or after the move-in date. This program does not provide assistance for more than one month of back rent, future or advance rent payments, late fees, or utility bills. Individuals requiring assistance beyond these limitations are encouraged to contact 211 for additional resources.

Provides housing problem-solving and/or one time financial assistance to assist individuals in remaining in safe and stable housing. Referrals through 211. If a CARE Center encounters an individual who is at imminent risk, will refer individual to 211 for referral. 211 provides triage tool.
