Provides permanent affordable housing solutions and vital support services to homeless and very low-income families and individuals in Contra Costa County. Partners with Mercy Housing to provide Permanent Supportive Housing for the most vulnerable at Garden Park Apartments. Also provides Permanent Supportive Housing in a multi-site program throughout the county (Richmond to Antioch). All Permanent Supportive Housing units are filled through the CC Coordinated Entry system. Homeless individuals can be placed on the Coordinated Entry list at the CARE Centers, homeless shelters or by dialing 211.

Offers three permanent supportive housing programs; Turningpoint Housing Program (THP), Permanent Step Program (PSP) and Project Thrive. All provide rent subsidy in which participants pay 30% of their gross income towards their rent. Clients must meet eligibility requirements and be referred through the coordinated entry system for housing openings.
