All services are free and provided by Family Health Advocates, Group Facilitators, Public Health Nurses and Mental Health Workers. Services include Interactive social support & empowering group sessions for pregnancy and postpartum, Nutrition, breastfeeding and infant care information, Prenatal yoga and meditation exercises, Linkage and referrals to community resource, Stress reduction techniques

Provides home visits, education, resource information and support to promote the health of pregnant women and their babies. Assists women in finding health coverage including Medi-Cal. Connects women to local prenatal providers and clinics. Educates and supports families about pregnancy and parenting. Links pregnant women and their families to appropriate resources and services.

Offers home visiting services for prenatal and postpartum families in Contra Costa County. Services include information about pregnancy, infant care, safety, monthly socializations, helping to navigate the medical system and community resources that may be of interest. Special focus on helping parents to understand child development over the first two years of child's life through a structured, home-based curriculum.

Offers an evidence-based long-term home visiting program that helps mothers with their first child. Due to the pandemic all visits are held online. Partners mothers with Public Health Nurses early in their pregnancy and offers two to four home visits per month that continue through her child’s second birthday. Public Health Nurses provide education to help women develop positive health behaviors during (and after) pregnancy, parenting education to support child health, development, and safety. Offer nursing assessments for early identification and monitoring of maternal and infant health status, encourages self-sufficiency by supporting parents attainment of their education and employment goals. Promotes supportive relationships among family and friends makes referrals to health and social service resources, in addition to care coordination with health and social service providers.
