Each client receives two interviewing outfits for interviews and a working wardrobe when jobs are obtained. Contracted by CalAIM to benefit Contra Costa Health Plan members, and provides 12 weeks of Medically Tailored Meals at no cost to individuals with chronic conditions. Offers a nutrition assessment by a registered dietitian who can prescribe 2 meals per day for 12 weeks. Provides three types of meals which include: fully prepared meals, semi-prepared meals and food boxes. Meals are delivered to clients home address and clients can select from a diversity of flavors and cultures.

Partners with the Contra Costa County Employment & Human Services Department (EHSD) and the County Health, Housing and Homeless Services Department (H3) to provide the HousingWORKS! Housing Support Program (HSP): HousingWORKS! provides supportive housing services to homeless CalWORKs families to help them achieve self-sufficiency and housing stability. Supportive services include assistance with housing search and temporary/permanent housing placement, time-limited subsidy assistance using Rapid Rehousing and Housing First models and on-demand in-home services. Eligible families are referred through EHSD and the H3 County Coordinated Entry System. Offers Youth and Young Adult (YYA) Housing Support program which provides time-limited subsidy assistance and in-home support services to transition age youth to help them achieve permanent housing and independence the CC County Coordinated Entry System.
