Offers eligible veteran families temporary financial assistance, outreach services, case management, and assistance in obtaining VA and other benefits which may include; health care services, daily living services, personal financial planning, transportation services, fiduciary and payee services, legal services, child care services, and housing counseling services.

Offers HUD-certified housing counseling. Works with individuals who wish to accomplish financial goals and become ready for home-ownership, manage rental obligations, or attain other financial and housing goals. Counseling services include one-on-one counseling sessions and group education workshops, pre-purchase counseling, credit repair, debt reduction, post-purchase financial readiness and budget counseling.

Clinical High Risk (CHR) program works with young people at risk for or experiencing very early symptoms of psychosis, prior to a first psychotic episode. First Episode Psychosis (FEP) program works with those who have had a first psychotic episode within the past year. Services are designed to support recovery and functioning and to prevent the occurrence of an initial or subsequent episode of psychosis. Offers individual, family and group therapy, care coordination, multifamily groups, employment and education support, occupational therapy, peer support, substance use counseling and psychiatric services.

Provides an adult day program for adults with developmental disabilities. This center serves severe to profoundly disabled individuals. Students participate in a multitude of learning and activities that promote self-expression, such as: arts and crafts, music and dance.

Provides information, referrals, assistance, understanding and hope to persons of all ages with disabilities as well as elderly individuals for the purpose of helping them to live as independently as possible. Services include: advocacy, benefits assistance, housing assistance, employment services, peer counseling services, assistance with independent educational planning (IEPs for school-aged youth), life skills, ADA information, public & businesses education. Referrals and resources are provided for: assistive technology, agencies serving the disabled, transportation, and youth programs. A small lending library of equipment is available.

Provides a food pantry for Pittsburg residents and needs based services which may include; emergency fund available unexpected situations, and vouchers for clothing that can be used in lieu of cash at St. Vincent de Paul thrift stores for discounted clothing and furniture.

Provides free surgical eye care for those individuals with no insurance and limited incomes. All clients must have a doctors referral. Areas covered include diagnostic testing, eye surgical care and eye medications. All services are performed in San Francisco at California Pacific Medical Center. The Foundation does not provide for routine eye exams, eye glasses, contact lenses or low vision aids.

Provides prenatal care, pill abortion services, birth control services, emergency contraception, HIV testing, HPV vaccines, LGBT services, men's health services, patient education, pregnancy testing, options & services, STD testing & treatment, and women's health services including breast and cervical cancer exams.

Transportation for medical appointments and other services in Walnut Creek are utilizing the ride sharing app Lyft. Apply by phone Monday-Friday between 9-10am the day before transportation is needed.

Provides dental services for Medi-Cal/Denti-Cal recipients. Services include x-ray, exams, cleaning, treatment, crowns, dentures and partials.
