Provides a broad range of services to adults with psychiatric disabilities. Issues include, depression, bipolar, schizophrenia, PTSD, anxiety, relationship conflict, developmental concerns, prescription management, family communication and parenting issues. We offer two programs: Contra Costa Outpatient Pittsburg and Community Support Program (CSP). Outpatient offers services for mild and moderate psychiatric disabilities including weekly individual and group psychotherapy. CSP offers intensive services for severe psychiatric disabilities including case management, medication management, 24/7 hour crisis line, individual/group/family psychotherapy, housing/education/vocational support, peer support, and dual diagnosis treatment.

Provides veterans financial assistance to entry of new housing or eviction prevention and Utility Assistance. The program also has potential to assist with past eviction debt on a case-by-case basis if it is a barrier to entry of new housing.

Provides suicide prevention and crisis hotline for Marin county residents. Counselors provide 24 hours immediate and confidential support.

Provides a variety of programs for families with young children, age 0-5 years. Information on HMG playgroups and various activities available on Facebook page. Offers virtual and in-person parenting classes and workshops, and activities for families such as art, music and more. Clients can meet with a community resource specialist and socialize with other parents. Center offers Kinder Readiness classes, ages and stages developmental questionnaire and HMG playgroup.

Provides a structured and safe transitional housing living environment for independent adults. Offers shared or private rooms and no credit checks needed.

Offers a supplemental nutrition program that helps low income pregnant women, mothers with infants, infants, and young children eat well, be active and stay healthy. Provides supplemental food benefits using a WIC card. Offers nutrition and breastfeeding support and education, and referrals to other health and community services. Offers a mobile-friendly application and website at m.wic.ca.gov for clients to find a WIC local agency and WIC grocers.

Provides comprehensive education and job training program. Students choose classes in the areas of Functional Academics, Community Independence, Independent Living Skills, Job Training and Social/Leisure Skills. Addresses the unique needs of adults with Asperger's Syndrome and related disorders transitioning to higher education and/or employment. Program provides a structured and supportive environment for students to develop social skills, increase independence, develop interests, explore resources, participate in the community, and create a social network of support. Provides re-cognitive training, memory skills development, independent living skills and career exploration to adults who have suffered an acquired brain injury. Assists individuals with ABI to relearn old skills, develop new coping skills and increase independence. Students may enroll in full day, morning, or afternoon only.

Provides a food pantry.

Provides ready-to-heat medically tailored meals, grocery boxes, and nutrition counseling to Medi-Cal members.

Provides volunteer opportunities to adults seeking to serve seniors 50+ with a variety of services ranging in duties and time commitments.

Provides low cost primary health care for adults, teens and children regardless of insurance or immigration status. Services include; immunizations, STD testing, and physicals. Provides comprehensive pregnancy services and well-baby care. In addition to prenatal visits, it includes education and support related to staying healthy during pregnancy, good nutrition, breast feeding, and psycho-social services, including psychiatric evaluations. Offers Covid testing and vaccination appointments.

Provides routine and preventive health care services including family practice, dermatology, gastroenterology, gynecology, high risk obstetrics, internal medicine, podiatry, surgery, pediatrics, prenatal, and women's health. Care coordinators relay messages to doctors and assist patients with medical concerns. Clients can reach their doctor's care coordinator by dialing the backdoor number and leave a message.

Provides public library services at 26 branches including a diverse selection of lendable materials including, books, eBooks, eAudiobooks, and DVD's. Branch libraries offer a variety of programs that encourage literacy for all ages, celebrate the arts & culture, feature local and nationally recognized authors and other speakers and performers. Other free programs and events include STEM (science-technology-engineering-art-math), performers, crafts, reading with dogs, movies, homework help, book clubs and games. Provides complaint service for individuals who have experience housing discrimination based on their race, color, national origin, religion, sex, family status, or disability; housing discrimination is illegal by federal law. If a person believes their rights have been violated in relation to renting or buying a home/apartment, they can file a fair housing complaint online or by phone.

Provides information and resources to protect public health and safety through the control and prevention of disease, injury and disability in the United States. Researches and provides information on infectious diseases, food borne pathogens, environmental health, occupational safety and health, health promotion, injury prevention and educational information to the community. Provides on the job training for individuals with employment barriers. Transitional employment program offers paid on the job skills training combined with workshops to develop personal and vocational growth. Programs offered: retail sales donation customer service material handling, processing of textile and hard goods, and truck driving. Sites are located througout Alameda, Contra Costa and Solano Counties.

Provides a wide variety of health care services for women that include free pap smears, breast exams, birth control and STD/HIV testing to all women who qualify.

Raises funds for Hospice of the East Bay and for patients who are uninsured or under-insured through direct donations, car donations, Appraisal and Estate sales and through five (6) thrift shop in Alamo, Concord, Danville, Martinez, Pleasant Hill and Walnut Creek.

Answers crisis hotline and warm line which supports at-risk seniors and adults with disabilities, family members and their advocates. Services include support, reassurance, counseling, crisis intervention, abuse prevention, medication reminders, well-being checks, and information and referral. Trained staff and volunteers receive calls and make outgoing calls to individuals who may be lonely, isolated, grieving, depressed, anxious, and/or thinking about death or suicide.

Provides HUD-subsidized independent living apartments, all-low income for seniors 62+.

Provides 15 to 20 pounds of fresh produce two times per month to qualifying households that are in need of food assistance. Produce is distributed from the Food Bank of Contra Costa and Solano Mobile Trucks.

Provides a food cards for Walnut Creek or Concord residents and needs based services which may include; utility and financial assistance, and vouchers for clothing that can be used in lieu of cash at St. Vincent de Paul thrift stores for discounted clothing or household/furniture items.

Provides prenatal care, pill abortion services, birth control services, emergency contraception, HIV testing, HPV vaccines, LGBT services, men's health services, patient education, pregnancy testing, options & services, STD testing & treatment, and women's health services including breast and cervical cancer exams.

Provides free legal services for Contra Costa seniors age 60+ including advice, counsel and representation in civil legal matters, such as: elder abuse, consumer matters, housing, income maintenance as well as advance health care directives and durable power of attorney. Provides fire suppression, emergency medical including paramedic, rescue, fire/arson investigation, building plan review, code enforcement, public education on fire and life safety, and weed abatement to Antioch, Clayton, Concord, Lafayette, Martinez, Pittsburg, Pleasant Hill, San Pablo, Walnut Creek, and most unincorporated areas of Contra Costa County.
