Provides a a virtual service where teens and young adults can get confidential one-to-one support sessions via video, audio or chat from a peer mentor of choice. Mentors are individuals who share clients identity, background, and/or lived experiences, and has been trained to help others going through something similar. Mentors (ages 18-35) come from very diverse backgrounds (over 50% identify as LGBTQ+ and/or BIPOC) and have firsthand experience with a wide range of mental health challenges ranging from anxiety and depression to addiction, trauma, eating disorders, evolving gender/sexual identity and more. Also offers peer-led group sessions on selected topics.

Provides a day health program licensed by CA Dept. of Public Health for individuals 18 and over with disabilities or multiple chronic. Program offers a combination of medical, social and therapy services for fragile adults while giving their caregivers and families respite. The day is designed to increase and/or maintain current physical and mental functioning. Also provides individualized plans combining medical, social and therapy service for adults with mid to late stage Alzheimer's disease and dementia. The day is filled with appropriate activities for people with memory impairments, and may include live music, physical horticulture and art therapies, exercise and games.

Provides comprehensive family planning services to eligible low income men and women who meet income guidelines and do not have insurance that covers family planning, or have insurance, but need to keep family planning services confidential, or have insurance but have not met the deductible or Medi-Cal Share of Cost, or have limited-scope Medi-Cal coverage that does not include family planning. Eligibility for teens is not based on parental income and by law parental consent is not required. Provides Enhanced Care Management (ECM) services for Medi-Cal members with highly complex needs. ECM is a benefit that provides extra services to help clients get the care needed to stay healthy. Qualified individuals will have their own care team, including a Lead Care Manager who will talk to client, doctors, specialists, pharmacists, case managers, social services providers and others to make sure everyone works together to get client the care needed. Lead Care Managers can also help clients find and apply for other services in their communities which include, medically tailored meals and housing supports.
