Enhanced Care Management services help to coordinate clients care. A Lead Care Manager will be assigned who will work with the patient, doctors, specialists, mental health providers, and other care staff to make sure everyone is working together. Lead Care Managers can also help clients find and apply for other services in their communities which include, medically tailored meals and housing supports, help patients understand and manage medications, help with scheduling appointments, find and apply for community services and work with patients and their family to improve their health.

Offers an early intervention program model that offers opportunities for individual learning within a natural group environment through the implementation of the Early Start Denver Model (ESDM) practices for children 18 months to 3 years with autism or at-risk for autism. Uses ESDM practices of daily routines and play-based activities to facilitate developmental sequences in young children with autism or at risk for autism similar to their typically developing peers. Aims to help minimize the developmental delays associated with early autism and increases young children’s developmental growth in the cognitive, social-emotional, communication, fine, and gross motor developmental areas. Service is provided in a classroom based environment and meets 5 days per week from 8:30am-12:00pm.
